CONSENT FOR THE USE OF IMAGE

I, the undersigned ………………………………………., pursuant to Article 81(1) of the Act on Copyright and Related Rights of February 4, 1994, hereby consent to the gratuitous use by Ms. Iga Majewska, conducting business under the brand "Warsaw Pilates Iga Majewska" (ul. Jana Kazimierza 12/35, 01-248 Warsaw, NIP: 821-256-38-35) and JOY IN ME S.C (ul. Ogrodowa 48/54, 00-876 Warsaw, NIP: 5272882838), of my image recorded in the form of photographs and video recordings, in connection with the workshops or training sessions conducted by Iga Majewska and JOY IN ME S.C. under the name “Trening pod chmurką” [Open-Air Training]. This consent is granted without territorial limitation, with the right for Iga Majewska to grant further consents to third parties, and includes:

  1. posting, making available, and disseminating photos or videos on the Internet (e.g., on Warsaw Pilates websites, on subscription platforms with online training, on the YouTube platform, and on social media) for the purpose of promoting the products, brand, and activities of Warsaw Pilates and the brands JOYINME, Cosibella Corner, Nick’s, Felix, and Mudii, which are partners of the aforementioned event; and

  2. posting, making available, and disseminating photos or videos in any promotional and informational materials of Warsaw Pilates, documents, posters, brochures, flyers, newspapers, magazines, and any other electronic and traditional large- and small-format forms, for the purpose of promoting the products, brand, and activities of Warsaw Pilates and the partner brands; and

  3. reproducing and fixing photos and recordings, including by printing, reprographic, magnetic, and digital recording techniques; and

  4. public display, screening, reproduction, as well as broadcasting and rebroadcasting of recordings and photos; and

  5. use of photos or video materials for the purposes of advertising and promotional campaigns for Warsaw Pilates and the sponsor brands.

Consent is granted for the entire duration of the promotional activities in connection with which the image was recorded.

Iga Majewska "Warsaw Pilates", as the controller of your personal data, informs you that the data will be processed for the purpose of promoting the activities of Warsaw Pilates. You have the right to access your data, rectify it, and demand its erasure or the restriction of its processing. Warsaw Pilates will respond to your request within 30 days of its submission and will cease processing the data, provided that there are grounds for doing so. Data may be transferred to the event sponsors..

Personal data (image) will be processed for the duration of the promotional or marketing activities in connection with which the image was recorded. If you believe that we are processing your data unlawfully, please contact us at kontakt@warsawpilates.pl. You also have the right to lodge a complaint regarding the processing of personal data with the President of the Personal Data Protection Office.

Consent may be withdrawn at any time by sending an email to the address kontakt@warsawpilates.pl or by correspondence to the controller's address.
